If you’re living with bipolar disorder, you know that the changes in mood can be unpredictable and disruptive. Periods of elevated energy or mania can give way to bouts of severe depression, with the unpredictability and severe symptoms making the condition difficult to manage. When medications and traditional therapy don’t provide sufficient relief, it’s natural to start exploring other options.
One such option is Transcranial Magnetic Stimulation (TMS), which is gaining the attention of patients and clinicians alike. While the research is still evolving, initial findings suggest that TMS for bipolar disorder may be effective in the management of bipolar depression. For many patients, this is the most difficult phase of the condition to treat.
TMS therapy is drug-free, non-invasive, and personalized to each patient’s brain activity and anatomy. At NeuroHealth Treatment Centers, we use advanced TMS technology to provide targeted care for patients with bipolar disorder who haven’t found success with conventional treatments.
Keep reading for more information on how TMS works, what the research says, and whether it may help you find relief from the symptoms of bipolar depression. Understanding Bipolar Disorder
Bipolar disorder is a complex condition where individuals alternate between episodes of extremely elevated mood (mania) and depressive symptoms. The duration and intensity of these episodes are difficult to predict, which can interfere with daily life.
There are several types of bipolar disorder:
- Bipolar I Disorder: Individuals with bipolar I experience one or more manic episodes that (a) last a minimum of seven days and (b) may require hospital care. These patients may also experience depressive symptoms prior to or after a manic phase.
- Bipolar II Disorder: The hallmark of bipolar II is hypomania (elevated mood and energy that is less disruptive than full-blown mania) interspersed with lengthy depressive episodes.
Manic episodes can be severely disruptive, potentially leading to impulsive decisions and poor judgment. For many patients, however, bipolar depression is more difficult to manage on a daily basis.
Bipolar depression is often characterized by diminished energy levels, lack of motivation, difficulty concentrating, and other symptoms that can affect quality of life and make day-to-day functioning difficult. Treatment resistance is reported in approximately one-quarter of bipolar depression cases, meaning medication and other standard interventions are ineffective at reducing symptoms.
Due to the severity of symptoms and prevalence of treatment-resistant bipolar depression, alternatives like TMS therapy are the subject of emerging research and being increasingly adopted as part of comprehensive psychological treatment for bipolar disorder.
Personalized Treatments, Backed by Science
We specialize in innovative, non-invasive treatments that offer real hope:
What Is TMS?
Transcranial magnetic stimulation uses an electromagnetic coil to deliver targeted magnetic pulses to specific areas of the brain involved in mood regulation. In individuals with depression and related mental health conditions, regions such as the left dorsolateral prefrontal cortex (DLPFC) are often underactive. By modulating neural activity, TMS can help to improve brain function and reduce symptoms.
During therapeutic sessions, the coil is placed gently against the scalp and positioned over targeted areas of the brain. The device delivers controlled magnetic pulses that pass through the skull and activate neurons. With repeated sessions, this stimulation can help improve communication within mood-regulating circuits of the brain and lead to a reduction in depressive symptoms.
No medication is required for TMS therapy, meaning there are none of the systemic side effects associated with mood stabilizers and antipsychotics. Treatment is performed on an outpatient basis without the need for anesthesia, so normal activities can be resumed immediately.
At NeuroHealth Treatment Centers, we have adopted the Nexstim SmartFocus® TMS device. This technology uses an MRI scan to precisely locate the left DLPFC and deliver highly targeted, individualized treatment.

How Effective Is TMS for Bipolar?
TMS therapy appears to be most effective at treating bipolar depression, rather than mania or hypomania. Research is ongoing, but several clinical studies support its use for this purpose.
A 2025 meta-analysis published in Biological Psychiatry Global Open Science reported response rates of approximately 47% and remission rates of 28% among patients with bipolar depression who received repetitive TMS (rTMS). These outcomes are similar to those seen in studies of TMS for major depression.
Earlier research has reinforced these findings. A 2023 meta-analysis published in Molecular Psychiatry found “novel evidence” that rTMS is effective in the treatment of bipolar depression.
A 2017 study in the journal of Neuropsychopharmacology examined the effectiveness of deep TMS in treating bipolar depression. The response and remission rates were roughly equivalent to those associated with rTMS in the later Biological Psychiatry study mentioned above.
TMS for bipolar disorder isn’t a one-size-fits-all treatment. Effectiveness can vary depending on a patient’s symptoms and treatment history, among other factors. Qualified mental health providers can help integrate TMS into a comprehensive treatment plan.

Is TMS Safe for Bipolar Disorder?
TMS therapy is generally considered safe for individuals with bipolar disorder, particularly when it is used to treat bipolar depression. Management by an experienced care team is vital for optimizing outcomes.
Multiple studies suggest that TMS carries a relatively low risk of mood switching (i.e., transitioning from depressive to manic or hypomanic phases). The risk is further minimized when clinicians conduct careful psychiatric screenings and observe proper treatment protocols.
In many cases, TMS is administered alongside mood-stabilizing medications. This can help reduce the likelihood of mood disturbances during treatment. Ongoing monitoring is essential for ensuring that patients with bipolar disorder tolerate TMS well and adjustments can be made.

Does TMS Have Side Effects?
The side effects of TMS are typically mild and short-lived. Potential side effects include:
- Scalp discomfort at the treatment site
- Mild headache
- Facial muscle twitching
- Dizziness
- Fatigue
For many patients, these side effects lessen or resolve after the first few sessions. Unlike medications used to treat bipolar disorder, TMS does not cause weight gain, drowsiness, or cognitive slowing.
Overall, TMS treatment is well-tolerated by the majority of patients with bipolar disorder and other psychological conditions.
Who Is a Good Candidate for TMS for Bipolar Disorder?
TMS therapy is not suitable for everyone. However, you may be a good candidate if:
- You are currently experiencing bipolar depression
- Medications and therapy have not provided sufficient relief
- You have difficulty tolerating the side effects of mood stabilizers, antipsychotics, or other psychiatric meds
As a rule, TMS treatment is used to treat depressive symptoms. It is generally not recommended if you are experiencing manic or hypomanic episodes. A comprehensive psychiatric evaluation is needed to determine if you are a good candidate based on your diagnosis, symptom pattern, and other treatments you’ve tried.
What to Expect During TMS Treatment
TMS therapy is an outpatient procedure that does not require sedation or anesthesia. Each session lasts approximately 20–40 minutes.
During each session, you will sit comfortably in our Fort Lauderdale office. One of our certified technicians will accurately position the coil against your scalp to target the left DLPFC. You may feel a light tapping sensation at the treatment site. Most patients are able to sit back and relax, with many using the time to read or listen to music or podcasts.
No downtime is necessary after TMS therapy. You can drive yourself home, go back to work, and resume regular activities without delay.
TMS treatment is usually administered five days a week over a period of 4–6 weeks. Symptom relief tends to progress gradually, though you may start to notice improvements after several sessions. Our clinical staff will monitor your progress and modify treatment to achieve maximum symptom relief.
Benefits and Limitations of TMS for Bipolar Disorder
The benefits of TMS treatment for bipolar disorder include:
- Treatment is non-invasive, with no anesthesia or sedation required
- Drug-free, with no systemic side effects
- Well-tolerated by most patients
- Growing clinical evidence supporting its use for bipolar depression
- No downtime after therapy sessions
It is also important to be aware of the potential limitations of TMS therapy:
- Not suitable for mania or hypomania
- Requires multiple treatment sessions
- TMS therapy is not FDA-approved for bipolar disorder
- Insurance coverage varies
- Minimal (but possible) risk of mania
Understanding how transcranial magnetic stimulation works and its application for bipolar disorder can help you make an informed decision about your mental health care. This is crucial for setting realistic expectations and getting the most from the treatment.
